It is important to keep your child up-to-date on immunizations they need for school or child care. Vaccinations play an important role in keeping your child healthy. Speak to your child’s medical provider or visit the WA State Department of Health.

A Health Passport

In partnership with the Brave Warrior Project, we offer a Health Passport to help families keep important health and care information in one place. The Health Passport includes medical information, communication needs, medications, emergency contacts, and healthcare providers. It also features an Emergency Sheet that can be kept in a backpack or car to help first responders, schools, and caregivers quickly understand a child or family member’s needs.
The passport can be used electronically or in print, shared with care teams, and updated annually. While not a legal document, it is a helpful tool for safer, more coordinated care—especially in emergencies.
Email nichole.franks@cdhd.wa.gov if you have questions. You can download the My Health Passport in English or Spanish at Related Documents - Chelan Douglas Health
